Outstanding Service Award 

The Birmingham, Alabama Chapter of CMSA grants the Outstanding Service Award annually in December.  This award is dedicated to members who have worked diligently on behalf of the chapter.  The award value will not exceed $1500 and is to be applied to expenses for the National CMSA conference in 2012. The application period is November 1, 2011-December 1, 2011.  Submission information is located on the attached application.
